What is Banded Plication?

With a typical adjustable gastric band surgery, the band itself is placed atop of the stomach, creating a new, smaller stomach pouch and restricting the passage of food. This procedure builds on that with the addition of gastric plication.

Here, the outside or greater curve of the stomach is folded over itself, and the band is placed directly atop of that, which enhances the effectiveness of either option performed individually. Banded plication therefore improves upon lap band surgery while retaining its advantages of minimal invasiveness comparative to alternatives such as gastric bypass.

Benefits of Banded Plication

Banded plication is new, but offers great promise for patients who choose this route. Only initial studies have been completed, but these have offered exciting results thus far, which is why Tristate NYBG is so excited to be involved at this stage.

The benefits of lap band with gastric plication include:

  • Banded plication requires fewer adjustments post-surgery than lap band alone. It also reduces the risk of band prolapse, or slippage, with lap band, which is one of the more common risks of complication with that procedure.
  • Banded plication is less invasive than gastric bypass or gastric sleeve. There is no cutting or stapling of the stomach, and the intestinal track remains unchanged.
What to Expect During & After your Procedure
  • Banded plication is a laparoscopic procedure, which means only minimal external incisions are needed.
  • You will be placed under anesthesia at the start of the procedure.
  • Small external incisions are created and the tools, band and laparoscope are inserted.
  • The tissue and vascular attachments to the stomach are partially cut along the great curve.
  • The adjustable band is inserted and attached via connecting tube to an access port.
  • The stomach is folded in on itself below the band.
  • Stitches are used to keep the fold in place, and the external incisions are sutured.

The entire process takes less than two hours to perform. Most commonly one overnight is required in the hospital.

What are the risks of the Banded Plication Procedure?

Any type of surgery or procedure involving anesthesia carries certain risks. Beyond those, banded plication risks include band or port infection, and band erosion. It’s important to note that this is a newer procedure, and therefore, there is not the same type of long-term data available as with other procedures.

It’s equally important to note here that gastric plication adds only minimal risk to the risk of the lap band procedure on its own, and is far less invasive, with lower risk, than gastric bypass or sleeve gastrectomy.

What type of diet will I need to follow after Banded Plication?

Your diet following a banded plication procedure will be similar to a post lap band surgery diet. This means you will move through three two-week phases after your procedure. The first will be based entirely on liquids. From there, you will add certain softer foods into your diet, and then a more complete range of solid and regular food choices.
